Forklift PM Schedule: Daily Checks & Service Intervals
A practical forklift PM schedule for daily examinations, hour-based service, battery care, defect follow-up, and maintenance history — with intervals you can adapt to each truck's manufacturer and duty cycle.
Why forklifts are different
Most equipment in your building fails on its own schedule and takes a production hit when it does. Forklifts are different for two reasons. First, they're regulated: OSHA 29 CFR 1910.178(q)(7) requires powered industrial trucks to be examined before being placed in service, at least daily, and after each shift when trucks are used around the clock. A truck with a condition that adversely affects safety must not be placed in service, and defects must be reported and corrected.
Second, a forklift failure is a safety event, not just downtime. A conveyor that stops costs you throughput. A forklift with failing brakes or a cracked fork heel is carrying 4,000 pounds at head height near your people. The failure modes that matter — brakes, forks, chains, tires, hydraulics — are exactly the ones that hurt someone when they let go. That changes the math on preventive maintenance: you're not just avoiding repair costs, you're managing the biggest struck-by and tip-over risk in the building.

Forklift PM schedule at a glance
Use this as a planning baseline, then replace the service intervals with the requirements in each truck's operator or maintenance manual. Hours, attachments, temperature, dust, washdown, floor conditions, and multi-shift duty can all justify more frequent work.

The OSHA daily examination
The daily (or pre-shift) inspection is the foundation of the whole program. It takes an operator 3–5 minutes and catches the majority of dangerous defects before they become incidents. Every truck, every shift it's used, before it moves a load. The core checks:
- Forks: cracks (especially at the heel), bent tips, worn positioning locks, uneven fork height
- Mast and chains: kinked or stretched lift chains, loose anchor pins, mast wear, smooth raise/lower travel
- Tires: chunking, tread wear past the wear line on cushion tires, pressure on pneumatics, debris embedded in the tread
- Brakes and steering: service brake, parking brake, and steering response, checked during a slow test drive
- Horn, lights, and alarms: horn works, backup alarm works, warning lights function
- Hydraulics: visible leaks, hose condition, cylinder drift under load
- Data plate and seatbelt: capacity plate legible, seatbelt intact and functional
Electric and internal combustion trucks add their own items. On electrics: battery connectors and cables for damage, electrolyte level, no exposed wiring, charger plug condition. On LP trucks: tank mounting and locking pin, hose and fitting condition, no smell of propane, relief valve pointing up. On diesel or gas trucks: engine oil, coolant, and a look for fuel leaks.
Two rules make the examination real instead of theater. One: make defects actionable — capture the truck, date, shift, condition, and person responsible for follow-up so a failed item does not disappear between operations and maintenance. OSHA's powered-industrial-truck standard does not itself require these examinations to be documented, but company policy, insurers, customers, or other applicable rules may impose recordkeeping requirements. Two: a condition that adversely affects safety keeps the truck out of service until it is corrected. A checklist that never routes defects to someone who can act is a signature ritual, not a control.

Beyond the daily check, forklift PMs run on the hour meter, not the calendar. A typical schedule looks like this:
Every ~250 hours: the standard PM. Engine oil and filter on IC trucks, chassis and mast lubrication, lift chain inspection and lube, tire and fork inspection with measurements, brake check, hydraulic fluid level, air filter. On electrics, this visit is lighter — grease, chain service, connector inspection — since there's no engine to service.
Every 500–1,000 hours: the deeper service. Hydraulic fluid and filter change, transmission or drive unit fluid, coolant system service, brake inspection with wear measurement, fuel filter, differential fluid. Check your OEM manual — Toyota, Hyster, Crown, and Raymond all publish interval charts, and warranty claims can hinge on following them.
Annually: a full inspection regardless of hours — fork wear measured with calipers (10% wear at the heel means replacement), chain stretch measurement, mast rollers, steering components, and a load test if your insurer or customers require one.
Why hours instead of months? Because usage varies enormously between trucks. A dock truck running two shifts can log 2,000+ hours a year — that's eight 250-hour PMs. A spare truck in the corner might log 300 hours in the same year and need two. Schedule both on a quarterly calendar and you'll over-service the spare while the workhorse runs 500 hours past due between visits. The hour meter is the truth about how hard each truck is working; use it. The practical version for most fleets is “every 250 hours or 90 days, whichever comes first,” so lightly used trucks still get seen a few times a year — seals dry out and batteries sulfate even when trucks sit.

If you run electric trucks with lead-acid batteries, the battery is a maintenance program of its own — and often the most neglected one. A traction battery costs $2,000–$6,000, is engineered for roughly 1,500 charge cycles, and poor care can cut its life in half.
Watering. Check electrolyte weekly and water after charging, not before — levels rise during the charge, and topping up beforehand causes acid overflow. Use distilled or deionized water and fill just above the plates. A plate that runs dry is permanently damaged.
Equalize charges. Run the charger's equalize cycle weekly or as the manufacturer specifies. It's a controlled overcharge that balances the cells and knocks sulfation off the plates. Skip it for a few months and weak cells drag the whole battery down.
Washing. Acid residue and dirt on top of the battery create a conductive film that slowly discharges it and corrodes connectors and the battery compartment. Wash batteries quarterly, or whenever you see white crust.
Opportunity charging. Plugging in during every break feels productive, but on standard lead-acid batteries each partial charge counts against the cycle life, and shallow charging without regular full cycles accelerates sulfation. Unless you've bought batteries and chargers designed for opportunity charging (or run lithium-ion, which doesn't care), stick to the discipline: run down to about 20–30%, charge fully, cool, then return to service.
Records that make defects traceable
A useful internal record lets you answer the operational questions quickly: who examined which truck, when, what condition was found, whether the truck was removed from service, and what was done before it returned. Keep operator-training records and maintenance records according to the requirements that apply to your organization; the daily-examination rule by itself does not create a federal documentation requirement.
This is where paper checklists fall apart. The clipboard sheets are illegible, coffee-stained, or missing the week you need. Nobody can prove the inspection happened before the shift rather than being pencil-whipped at the end of the month. And worst of all, paper has no follow-up: an operator writes “brakes soft” on Tuesday's sheet, the sheet goes in a binder, and nobody with the authority to fix it ever sees it. Three weeks later the same truck is still on the floor. A defect that was reported and ignored is far worse in an audit — or a lawsuit — than one that was never found, because now there's a paper trail showing you knew.
Digital records fix all three failures at once: timestamped entries prove when the check happened, nothing gets lost, and a failed item can be routed to someone who has to act on it.
Setting it up
A working forklift program takes about a week to stand up:
- List every truck with make, model, serial, fuel type, and current hour meter reading. Include the rentals and the spare nobody talks about.
- Load OEM intervals for each model — the 250-hour and 500–1,000-hour services and the annual inspection — as hour-meter-based PM schedules.
- Assign the daily check per shift. Each operator completes the checklist on the truck they're taking, before the first load, with the hour meter reading captured as one of the items.
- Route defects into work orders. Any failed checklist item should create a work order automatically and, for safety items, tag the truck out of service. This is the step that turns inspections from paperwork into prevention.
From there the routine is easier to manage: your team can enter meter readings, review PMs that trigger from those readings, track defects through repair, and retrieve the recorded inspection history for a truck without digging through a binder. The team still needs to monitor due work and verify that inspections and repairs are complete.
